Capital Campaign
Family Service and Guidance Center's “Hearts Full of Hope Campaign” is raising funds with which FSGC will build the Child and Family Academy, 22,000-square foot, two-story facility designed for the provision of specialized mental health services to children from birth to age 6 and their families.

Independent Living!
The Independent Living for Teenagers group is targeted to young adults, age 15 to 21, with various disabilities who are interested in becoming independent. This eight week program teaches skills which will assist them in an independent lifestyle.

Child Welfare Contractors - Admission Information
Family Service and Guidance Center is very pleased that you have selected our agency to serve your needs. Our process begins with the intake appointment that you have arranged with the Intake Coordinator. Prior to your scheduled appointment please print and complete the client admission forms listed below and bring them with you to your intake appointment.

Family Service & Guidance Center is located at 325 SW Frazier, Topeka , KS.

When should you arrive?
•  After contacting our office please make note of the date and time for your appointment along with the name of the clinician that you have been scheduled to meet with. Please allow for a 2.5 hour visit.
•  If you will be late, please call to determine if there is still enough time to complete the appointment. If not, we will be happy to reschedule.
•  If you cannot attend the scheduled appointment, please call ASAP even if it is after the scheduled time.

Who must attend?
The client and foster care/adoption worker must accompany the client to the appointment.

What is the process?

•  Please check in at the front desk when you arrive.
•  You will meet with an Intake Coordinator to finalize paperwork.
•  You will meet with the clinician for the evaluation interview and treatment planning.
•  Schedule a follow up appointment if requested.

In addition to the forms listed above, you will need to bring:
•  Completed Child Behavior Checklist (CBCL). We are unable to house this form on the website, but you should have copies in your office.
•  Previous Treatment Information – Reports from previous providers are always helpful.
•  Copy of the insured's insurance card.
•  Medication bottles
