Admission Information
Family Service and Guidance Center is very pleased that you have selected our agency to serve your needs. Our process begins with the admission appointment that you have arranged with the Admission Coordinator.
Where are we located?
Your appointment could be scheduled at 325 SW Frazier or 327 SW Frazier. Please make note of the location when arranging the appointment with the Admission Coordinator.

When should you arrive?
- After contacting our office please make note of the date and time for your appointment along with the name of the clinician that you have been scheduled to meet with. Please allow for a 2.5 hour visit.
- If you will be late, please call to determine if there is still enough time to complete the appointment. If not, we will be happy to reschedule.
- If you cannot attend the scheduled appointment, please call ASAP even if it is after the scheduled time.

Who should attend?
The client should always be present and at least one legal guardian. If the child is in foster care and the foster care worker is located in the Topeka vicinity, they should attend in person. If not, they should be available by phone.
What is the process?
- Please check in at the front desk when you arrive. The receptionist will review your paperwork and copy your insurance or medical card.
--You may access the majority of the admission forms online using the links below
- You will meet with an Admission Coordinator to finalize paperwork and answer any financial questions that you have. If time does not permit, you may need to return after your session with the clinician.
- You will meet with the clinician for the evaluation interview and treatment planning
- Schedule a follow up appointment if requested
