How Do I Start?

Get started at FSGC in a few easy steps

Step1 out of 6 steps

There are two options to “Pre-Register.”

  • Call 785.232.5005 and ask to speak to Admissions. An Admissions Specialist will tell you everything you need to do and answer all your questions.
  • Online – Click here. Once you’ve completed the form, be sure to click “Submit” to send it to us. FSGC Admissions will contact you to set up your appointment. Family Service & Guidance Center has an Admissions Clinic that allows your child to be seen the same day you make your first phone call or walk through the doors. More information below.

Step2 out of 6 steps

Click “Admissions Paperwork” and “Print.” Complete and sign the documents and bring them to your child’s first appointment. Doing this before you come to FSGC will save time. If you need help filling out the forms or have questions, call Admissions. If you have problems downloading or printing the paperwork, call Admissions and ask for the forms to be mailed to you.

Step3 out of 6 steps

If your child has received mental health care services before or if they are currently involved in treatment, contact their providers and ask them to release any records to Family Service & Guidance Center, Attention: Admissions. You may also bring them with you to the first appointment, if you like.

Step4 out of 6 steps

Be sure to bring your child’s insurance card with you to the appointment. If you do not have the card, try to get a copy of the card, front and back.

Step5 out of 6 steps

FSGC provides a sliding fee scale based on household income if you live in Shawnee County. If you wish to pay on a sliding scale, be sure to bring income verification. Examples of income include:

  • Wages (copy of recent check stub or most recent tax return)
  • Social Security Benefits
  • Public Assistance (Medicaid)
  • Retirement/pension (SSI/SSDI) Child Support/Alimony – received or provided
  • Unemployment Insurance
  • Other Income not listed here.

What time should you arrive at FSGC?

If you have made an appointment

Admissions paperwork is complete

  • Please arrive at least 15 minutes before your scheduled appointment.

 

Admissions paperwork is not complete

  • Please arrive 45 minutes before your scheduled appointment.

 

We encourage you to phone ahead or pre-register for children’s mental health services in Topeka, if possible. This helps ensure the availability of FSGC staff based on your insurance coverage.

If you have not made an appointment*

Admissions paperwork is complete

  • To be seen in the morning, you should arrive by 10:30 a.m.
  • To be seen in the afternoon, you should arrive by 1:30 p.m.

 

Admissions paperwork is not complete

  • To be seen in the morning, you should arrive by 10:15 a.m.
  • To be seen in the afternoon, you should arrive by 1:15 p.m.

  • For the first appointment, the child and at least one parent or legal guardian must attend.
  • Please check in at the front desk when you arrive. The receptionist will review your paperwork and make a copy of your insurance or medical card and identification.
  • At your first appointment, you will…

a.  Meet with an Admissions Specialist to finalize paperwork and ask your questions.

b.  You will meet with a therapist at the initial appointment and discuss your child’s treatment options.

c.  Schedule a second appointment, if one is needed.
